Appointments:
Patients are seen by appointment. Walk-ins are difficult to accommodate with the high volume of existing patients so please call The Center to check availability. To make an appointment, please call 404-523-6571. Spanish and Portuguese interpreters are available to those who do not speak English but it is recommended that you bring an interpreter with you if you have one available.
Payment:
Charges are calculated from a sliding scale based on income and family size. If you are a first time patient you will need to bring photo identification, $50 and proof of income (W2 or 3 consecutive paycheck stubs) to determine your payment. W-2's, current paycheck stubs, unemployment letters from the unemployment office or letters from employers are all suitable as proof of income.
Patients residing in rehabilitation centers or shelters must provide a dated (for the day of service) letter confirming their residency and employment status. Patients who are truly destitute will be seen free of charge.
